Hi.
First just for clarity a Download consists of multiple parts, most of which are optional. A Download has a name and may have associated description, images, a video or audio preview, and a downloadable file.  All parts of the Download except the downloadable file are on the main host.  The downloadable file may be located on a separate (external) serve.

So in principle the link is to the Download not just to the downloadable file but one can set up the link to be in effect a Direct link to the file. Please see the article
http://www.jdownloads.net/documentations/item/download-actions-direct-via-summary-page-or-via-download-details
which may help you.
If you have the files on a separate server then the actual downloading depends on that server.

Hi
I think perhaps I now understand your problem! :)    I think you are saying is all that you want to see is the actual title as a link. That is you do not want to see any description, previews or images buttons or the like.

Firstly there is no problem in having the files part of the Download located on another server as an external link as you already know.

The way one sees Downloads on the screen is controlled by the layouts you activate.  There some Layouts that would suit your needs in the Files Layouts. One is "Files link only v3.2".  This literally just shows the title as a link.  You will need to set up according to article I mentioned earlier.  Think you want the 'No Yes Yes' option.  If you use that one  with say List All Downloads menu item it is "messy" but there are others.
http://www.jdownloads.net/documentations/item/download-actions-direct-via-summary-page-or-via-download-details

Other Layouts which might be useful are 'Files link and icon v3.2' and 'Files Layout Single Line v3.2'

If you use a menu item like List All Categories then users can get to the Downloads that way. In that situation the 'Files Layout Single Line v3.2' is probably best.

Alternatively you might wish to see the download link in the middle of some text and that uses the Content Plugin.  For more details on the Content plugin see articles below which use the 'Files link and icon v3.2' and the 'Files link only v3.2' layouts
http://www.jdownloads.net/documentations/item/using-the-content-plugin-in-articles
http://www.jdownloads.net/documentations/item/overview-and-installation-notes-for-jdownloads-plugin

It is straightforward to generate a new layout by doing a 'Save and Copy' - see
http://www.jdownloads.net/documentations/item/editing-a-layout
http://www.jdownloads.net/documentations/item/layouts-overview

Finally if you send me a picture indicating any changes in the layouts then I will help you create a suitable one.

Colin
PS At present we are creating a Layout Library with many more layout possibilities.
